Enrollment Process

To get started, please give us a call or complete the intake application and email it to one of our Family Services Department for follow-up within 24 hours.

The following documents are required to enroll or place your child on our waitlist:

-Intake Application

-Birth Certificates for children who live in your household under 18 years old

-One month's proof of income

-Proof of address

-Employment Verification

-Immunizations

-Physical
